Surgical Scar Tissue: How Acupuncture Can Help
Every surgery, no matter how carefully performed, creates trauma to the body's tissues. An incision cuts through skin, fascia, muscles, blood vessels, nerves, and connective tissue. As the body repairs itself, it lays down collagen to close the wound. This new tissue—known as scar tissue—is an essential part of healing.
Ideally, scar tissue gradually remodels into flexible, healthy connective tissue. Sometimes, however, this remodeling process is incomplete. The scar remains dense, adheres to surrounding tissues, and limits normal movement. Instead of becoming part of healthy tissue, it becomes a source of ongoing mechanical stress. From a rehabilitation perspective, this is a form of microtrauma.
A restricted surgical scar changes the way muscles, tendons, and fascia move. As nearby tissues compensate for this loss of mobility, they are subjected to thousands of small, repetitive strains during everyday activities. The Chinese Medicine Perspective
Classical Chinese medicine has described this process for more than two thousand years. Trauma interrupts the normal circulation of Qi and Blood through the acupuncture channels. Once the skin closes, healing is not considered complete if circulation remains impaired. The remaining restriction is traditionally described as Blood Stasis (Yu Xue)—a condition characterized by fixed pain, hardness, adhesions, numbness, and limited movement. The goal of treatment is to restore the free circulation of Qi and Blood so that the body can continue remodeling the scar and restore normal tissue function.
One of the oldest acupuncture methods for treating scars is Surround the Dragon (Wei Ci). Fine needles are placed around the scar rather than directly into it, stimulating circulation in the surrounding tissue while helping soften adhesions and improve mobility. Depending on the patient, treatment may also include classical acupuncture, electroacupuncture, motor-point acupuncture, and gentle manual scar mobilization.

Laser Acupuncture for Surgical Scars
Some scars remain too sensitive for traditional needling or are located in cosmetically delicate areas.
In these cases, laser acupuncture provides an excellent alternative. Instead of needles, therapeutic light is applied to acupuncture points and the tissues surrounding the scar. This process, known as photobiomodulation, stimulates cellular energy production within the mitochondria, helping regulate inflammation and supporting the body's natural tissue repair processes.
Class IV therapeutic lasers can penetrate several centimeters into soft tissue, making them particularly useful for deeper surgical scars involving fascia, tendons, muscles, and joints. Often I combine laser acupuncture with traditional acupuncture to encourage both local tissue healing and restoration of the affected acupuncture channels.

What the Research Shows
Although more large clinical trials are needed, current research is encouraging.
- A 2019 review by Tuckey and colleagues found that published clinical studies consistently reported improvements in scar pain, itching, and scar quality following acupuncture, although the overall quality of evidence remains limited because of small study sizes and differing treatment protocols. - A 2024 systematic review published in the Journal of Clinical Medicine concluded that acupuncture and dry needling may improve scar pain, tissue mobility, and quality of life in patients with symptomatic scars. The authors also emphasized the need for additional high-quality randomized clinical trials to establish standardized treatment protocols. (PMC)
- Research on therapeutic laser treatment is even more extensive. Experimental and clinical studies suggest that photobiomodulation may reduce inflammation, improve collagen organization, stimulate cellular energy production, and support wound healing, making it a valuable complement to acupuncture during postoperative rehabilitation. A surgical scar is part of a larger network of muscles, fascia, nerves, and connective tissue that must move freely for the body to function at its best.
